How do I transfer my Chinese Visa on to my new passport?

The following is the latest information (January 2012) from Public
Security Bureau (PSB) – Entry and Exit Administration regarding transfer
of your Chinese visa on to your new passports:

1. Foreign passports with a Chinese Residence Permit: Chinese
Residence Permit MUST be transferred to the new passport(s) in 10
calendar days. Please present the yellow envelope with a stamp date
from the Embassy to PSB.
2. Replacement passports for lost or stolen passports:
Passports holders must apply for a new visa in 10 CALENDAR DAYS from the
date of issuance of the new replacement passports.
3. Foreign passports with other types of visas: Chinese visas
should be transferred to the new passports before the visas expire, the
10-day requirement does not apply, if passports holders will remain in
China for “some time” and before the visa expiration date, or have
multiple entry Chinese visa. The canceled old passport and new passport
should be used simultaneously when exiting and entering China. Remember
to transfer the Chinese Visa on to the new passport before the visa
expires.
4. Visa(s) may not be transferred to the new passport if PSB
determines that applicants are not returning to China in the same visa
category listed in the old passport. Old and new passports should be
used simultaneously when exiting the country. A new visa in applicable
category must be obtained prior to returning to China
